Does Immunotherapy Work for Colorectal Cancer?
Checkpoint immunotherapy can be a major option when colorectal cancer is MSI-H or dMMR. That means mismatch repair is not working and the tumor carries many mutations. It is a minority of metastatic cases. MSS or pMMR disease, which is most colorectal cancer, does not get the same result from a checkpoint inhibitor. An investigational combination, including a new bispecific antibody that has only trial permission, is not established routine care. The biomarker comes before the product name.
Applicability: this answer is about checkpoint immunotherapy. A cell therapy, a vaccine, or a bispecific antibody in a trial is a different product and needs its own evidence.
Read the biomarker before the product
MSI-H means microsatellite instability-high. dMMR means mismatch-repair deficiency. They point to the same biology in this setting: the tumor’s repair system is broken. MSS means microsatellite stable. pMMR means mismatch repair is intact. Most metastatic colorectal cancers are MSS or pMMR.
MSI-H or dMMR
In this group, checkpoint immunotherapy may be a major treatment option, including in metastatic first-line care. Cao and Jiang, in the 1 October 2026 interpretation, say nivolumab alone or with ipilimumab was added as a first-line grade I recommendation in metastatic disease. That sentence does not itself write MSI-H. This page does not extend it to MSS or pMMR cancer, and it does not treat every PD-1 product in China as interchangeable. The same interpretation adds FOLFOX plus atezolizumab as an adjuvant option for stage III dMMR colon cancer, and it separates upfront surgery from neoadjuvant immunotherapy in non-metastatic MSI-H or dMMR colon cancer. Those are guideline sentences, not a product menu.
MSS or pMMR
Checkpoint immunotherapy is not automatically equivalent in MSS or pMMR colorectal cancer. Investigational combinations must not be presented as established routine care. The August 2026 IND for opamtistomig, a PD-L1/4-1BB bispecific antibody, allows a phase Ib/II combination trial in metastatic colorectal cancer. It does not approve a treatment. What is MSI-H or dMMR colorectal cancer?
MSI-H means microsatellite instability-high. dMMR means the mismatch-repair proteins are deficient. In colorectal cancer these findings mark the group in which checkpoint immunotherapy may be a major option. MSS and pMMR are the opposite findings and are not treated with the same assumption.
